Kindergarten Lead Letter
This form is required by the St. Louis County Health Department for all incoming Kindergarten students.

Physical Form 
This form is to take with you for your child's annual physical.  We are required to have a signed and updated physical form for all Kindergarten, Third, Sixth, and new students entering St. Peter.  If your health provider has a standard form their office uses, we will accept that as well.

COVID INFORMATION

Please Note: the onset of Covid 19 infection can present with very insidious, minor symptoms. Please take these seriously. If anyone in your household develops any COVID like symptoms, consider keeping everyone home until the affected person can be observed longer and/or be evaluated by a healthcare provider.
